INTEROFFICE MEMORANDUM – MATERNITY LEAVE

For many of us, the ultimate gift is the feeling we get when looking into a small child’s eyes.  Somewhere in that sparkle, there is a little part of you and your heritage, and there will be so much for the child to see as life unfolds.  Birth is nothing less than a miracle.

Amid the exciting preparations, my professional career aspirations have become stronger than ever.  Like my parents and their parents, I want my child to have nothing but the best experiences and opportunities.

I am requesting a maternity leave that, barring any unforeseen circumstances, will last from [DATE THE LEAVE BEGINS] through [DATE
THE LEAVE ENDS].  This bonding period is necessary for our family.  In
addition, plans must be made to assimilate an infant’s needs gracefully into our professional lives.

You have my home telephone number; please feel free to call with even the smallest question.  I look forward to sharing this gift of life with my [COMPANY NAME] family.  If not in person, I promise to bring in my share of those “Look at my beautiful baby” pictures.

INTEROFFICE MEMORANDUM – LEAVE OF ABSENCE REQUEST FOR FAMILY REASON

Families cannot be replaced.  Possessions and wealth lose their meaning when measured against the joy of holding a loved one’s hand or welcoming a new generation into the world.  To believe otherwise is to mock humanity’s most basic instincts.

Being an important member of a family is a source of tremendous happiness and many responsibilities.  I have been forced to make a decision recently between career and family priorities because my [RELATIONSHIP OF PERSON NEEDING HELP] needs my undivided attention. After exploring various solutions, I believe I am the best one to carry this burden.

The leave, if approved, would be for [TOTAL NUMBER OF LEAVE DAYS
REQUESTED] days beginning on [DATE THE LEAVE BEGINS].  Therefore, the
date of my return to work date would be [DATE THE LEAVE ENDS].  I
realize that this may be an inopportune time, and I will understand if
you should decide not to grant my family’s wish.  Please advise me of
your decision at your earliest convenience.  Thank you for your consideration.  Again, I will understand if you believe that the
burden imposed by the leave would be too great for my other family,
[COMPANY NAME], to bear right now.

INTEROFFICE MEMORANDUM – LEAVE – MEDICAL REASONS

In our day-to-day existence, we tend to take things for granted unless something out of the ordinary forces us to sit up and take notice.  The car that starts in the morning and the copy machine that works are just two examples.  Until now, I had never given much thought to what I would do if for some reason my health was temporarily taken from me.

The right food, proper exercise, and an active mind has served me well over the years.  Today, however, I find myself in a precarious position.  Despite my desire to continue doing my job with all the dedication and energy I normally put forth, health limitations have made it impossible for me to do so.

My friends and physicians tell me I am pushing the healing process, which invariably does the opposite and lengthens the cycle. Therefore, for our mutual benefit, I would like to take a medical leave of absence from [DATE THE LEAVE BEGINS] through [DATE THE LEAVE ENDS].

I expect my health problem caused by [HEALTH REASON FOR LEAVE] to
be completely under control before returning to work with renewed
vitality.  Thank you for understanding that this leave is necessary
for [COMPANY NAME] to have this team player back in full force.
